In order to receive consideration for a donation, an organization must be eligible and must submit their written request on event, organization or company letterhead.

All Requests must include:
- Description / mission of the organization
- Date, time and location of event
- Event purpose / beneficiary
- Verification of EIN#
- Contact name, telephone number and complete street address
Please understand completion of the letter does not guarantee HEADstrong Foundation will fulfill the request. All requests must be made at least 4-6 weeks in advance.
